Poutiri Pahi Mobile Health Service

To support you to stay well, get well and live well Poutiri provides a mobile free health clinics closer to home.

The Pahi provides:

  • Children’s health checks

  • Hearing & vision checks

  • Oral health

  • Heart checks

  • Diabetes checks

  • Bowel screening

  • Vaccinations

  • This service is free, we welcome a koha of non-perishable food for Pataka @ 74 Jellicoe Street.

KMANS Chronic Care Nusing Service

Has your health been affected by diabetes, cardiovascular or respiratory disease?

Do you understand the effects of these conditions?

How well are you managing your health?

  • We can support you (and your whānau) by helping you understand your condition, explaining what your medications are for, addressing any health concerns you may have, working alongside your GP/nurse and supporting your whānau with your healthcare.

  • We can see you at Poutiri Wellness Centre, at home, clinics held at local Marae or by phone.

  • This service is free, we welcome a koha of non-perishable food for Pataka @ 74 Jellicoe Street
